Clarify what "cat" documentation means by "blank" lines.

* doc/coreutils.texi (cat invocation): "Blank" lines actually mean
empty lines.
* src/cat.c (usage): Say that "nonblank" means nonempty.  Clarify
--squeeze-blank.
