New option: --all-repeated (-D).
(output_all_repeated) [output_mode]: New enum value. (usage): Describe it. (writeline): Test for new mode. (check_file): Likewise. Based on patches from Jochen Hein and Florin Iucha. (main): Diagnose `too many arguments'.
